7th Grade Immunizations
By 7th grade, students will need to have two additional vaccines: MCV4 (Menningococcal) and Tdap (Diptheria, Tetanus, Pertussis). Please see the letter linked below (it includes a form to fill out). If you choose not to vaccinate, please fill out the Legal Immunization Exemption Form. Additional information about Ohio Required Immunizations can be found here.
